Coral Died, another one going... ideas?

Had a Birdsnest die rather quickly, all else okedokee??

pH 8.0 - 8.2
Salt 1.025
Kh 10.2 - 10.6
Ca 430 - 450
Mg 1200 - 1280
Po4 .05 - .10

This is a pic before it all died.
I did a 10 min dip in Coral RX after this pic. seemed to slow down the decay, but didnt stop it. Also 25 gal W/C

Any ideas?
There is another coral that looks like it may be affected as well?

Looks like RTN. May want to grab a few healthy frags above the dead part and hopefully one of them survives. Have you changed any equipment?

Thanks for the reply.....

No changes to the tank in 3 months.
I was out of town for 14 days, Had a Kh spike last month to 13.2 on 10/8/12 This was a result of Ca Reactor adjustment. 25gal Water Change & Replaced GFO & Carbon on 10/9, the Kh was 10.4. The tank usually hangs in the 10's.. as tested with Red Sea Pro Kh Test

Had a Po4 Spike to .18. on 10/18/12, Not sure why.
Water Change, GFO & Carbon had it down to .09 (Tested with Hanna) by 10/23.

Did another 25 Gal Water Change 10/27

All parameters have been stable since ???

PS... Everything else in the hank looks wonderful!

**** Carbon dosed is RX08 (Bulk Reef Supply) 1 cup as per their calculator. GFO dosed High Cap. .5 cup. (.5 cup, but replaced every 2 weeks with carbon instead of 1 cup every month) in a dual BRS filter.
