If I'm reading this right, your salinity is at 25ppt? Which is low, I'd raise it asap to 33 - 35ppt. But I'd do it gradually overtime, don't rush it.

could it be possibly because we do 25% water changes every 1.5 weeks?

I change 5 gallons once a week in my 55, and in a smaller tank the more the better....
It seems to me that the SG is to low, it should be around 35ppt (1.025 sg)....

I doubt keeping up keeping up with WC is leading to any problem. I'd make sure when you do WC that your top off water is close to the parameters of the DT. You don't want a huge swing in parameters, that will stress your corals out and possibly even death.
