Internal drain

I have always used the inside weir with the outside pipes. How does the inside weir drain work when the pipes are going through the base of the aquarium? I get the overflow part but what about the water that is in some cases over 20" below the weir. Doesn't it get stagnant?

Constant flow into the box moves the water around.

It can certainly get stagnant.. Many throw a small powerhead down there..
Just another "downside" to that type of overflow setup.

That was my thought. There is no way the water at the bottom of that box gets moved to the top. Don't hear much about that issue.

I believe a lot of people with the reef ready tanks have set it up as a herbie, in which case the syphon can be lower in the box. If the sump can handle the backflow then u can pull the water all the way from the bottom of the overflow box

It really isn't a problem in all but the lowest flow systems. In my dual corner flow tank, I can see a gentle flow of water in the bottom of each overflow. I can tell because the many feather duster worms that grow down there are moving. Drop a powerhead if you must, it isn't necessary though.
