coral question

I have a large acro that has begun to lose large amounts of tissue from the base up. The base still has all its tissue but the coral is losing tissue up and along the branches.
Ca is 405
Nitrates 10ppm
nitrites 0
alk 4 meq
ph 8.3
po4 .05
It is in high flow and close to the top under 250 watts on a lumantek ballast 12k reeflux bulb.
What should I do to make the tissue loss stop?
