Is there another fish harassing him possibly?

Thanks Rob, No not that I can tell I have a flame angel, Purple fire fish (the only one that really hangs on that side of the tank), Indigo Pseudochromis, and a kings Damsel. Everybody eats and he still comes way out of his hole to grab food so not really a problem there. System has been up for more then a year. Could the flow be too much for him? I am all out of ideas. Thanks Tony

get him a pistal shrimp, so he can build him a cave which they can both live in making him feel more safer and less stressed out. ( if stressed out could be the case)

Thanks Dirtydeeds I will try that. I have been trying to explore every option I can think of. It's either stress like you suggested or aggression that is happening when I turn the lights off. If I had to take a guess I would think its the flame angel but not sure why he would bother a fish that doesn't compete with him on any level. Time will tell I guess I haven't lost a fish in almost a year.
