Very sick tang in QT

Purchased a powder brown tang about a week and a half ago and I have had him in QT ever since. Yesterday I noticed some discoloration on part of his body now I get home today and he looks like this. Is it ICH, parasites?

I'm seeing patchiness which could be bacterial, but I'm also seeing one discrete ich cyst, the white dot; and his swimming indicates he's oxygen-short, so it's in his gills. Immediately start adding fresh water to lower that salinity to 1.009 exactly, over the next 48 hours. Hopefully we're in time and you may see some improvement in a matter of hours as that salinity drop makes it more comfortable for him and more life-threatening for the parasite. Change your filter---no sponges: floss, and toss it every time it stains. Up the oxygen as much as possible: don't hit HIM with bubbles, but a bubble wand might help out. TO do this accurately you'll need a refractometer or really excellent luck getting a hydrometer to read right. You need to be bang-on, and draw a line on your qt marking the fill, and top off morning and night to make sure you stay on that mark: ideally, you'd have a separate ATO for the qt. Leave him in hypo until 4 weeks after the LAST sighting of a mark or cyst.

Salinity 1.024-6; alkalinity 8.3-9.3 on KH scale; calcium 420; magnesium 1300, temp 78-80, nitrate .2. Ammonia 0. No filters: lps tank. Alk and cal won't rise if mg is low.

Current Tank Info: 105g AquaVim wedge, yellow tang, sailfin blenny,royal gramma, ocellaris clown pair, yellow watchman, 100 microceriths, 25 tiny hermits, a 4" conch, 1" nassarius, recovering from 2 year hiatus with daily water change of 10%.

What is your ammonia and nitrite readings?

definitely track those: if ammonia shows, use Amquel or Ammolock. One bennie of going with hypo---you don't have to sweat interactions.
