You may be ok quarantining 4 fish in the 20 gallon QT. Is it a 20 Long, by any chance? If you have trouble keeping the 4 fish in that QT, then you might be able to borrow another tank for a while, use a rubbermaid container of some type, or buy a second QT during a Petco tank sale for $1 per gallon. It will be difficult to juggle cycling and the additional equipment, but you may be ok with a second QT if you are in the hobby for the long term.
This I agree with. There is no way to tell where the Ich came from. Your friend may have been absolutely right in that there were no Ich in the tangs, and the Ich may have been in the clowns. Or even in your tank with the liverock. No one can say for sure now.

I use a 20 gal as a separate refugium. Among my fish are 4 tangs. I would have to say no way can you keep an appropriate hospital tank using a 20 gallon for 2 tangs and 2 clowns. They will have to be treated for a mimum of 4 weeks, and likely be in that tank for 6 to 8 weeks. No way in a 20 gal, IMO.

+1

I have one tang in a 29 gallon QT and he seems a little cramped.

With my personal experience, i would leave them in the DT. I have a purple tang who is crazy! Everytime i move rocks or move something in my tank, she stresses out and gets ICH for a couple of days but she never stops eating. I feed brine shrimp soaked in garlic. As long as the fish dont stop eating and are healthy, they should be able to overcome ICH. I have a pair of clowns in with the purple tang and they never catch ICH when she has out breaks.

So try just feeding them and keeping them in good health. I would start to worry if they stop eating. Ive dealth with ICH with quite a bit of my fish and once they stop eating is when they're really going down hill but like i said as long as they eat like pigs like you described, they should be able to get through it. Get some garlic and start soaking your food. If giving them proper nutrition doesnt work, then a QT is in order.

On a side note, when i started with the whole ich thing, i was to the point where my fish wouldnt eat, so i did a QT. A sailfin tang and a yellow tang.... in a 10 gallon. Im ashamed of it because thats no where near the space they need but thats all i had at the time. My yellow didnt make it but my sailfin tang is better than ever. And they were reallllyyy bad... Fins were torn, they were all skinny, and not because they were fighting, but because of the ICH. and like i said, my sailfin is now my fattest fish that i have. I had him in the QT for maybe 2 weeks if that, i put him back into the original DT and everything has been good ever since. This is my personal experience though, it might turnout different for someone else.
