Sick tang

I have a sailfin tang that is appromitaly six months old. It has sunken white spots on its head. It's colors have faded and gotten dark. It is just sitting at the bottom of the tank barely breathing. None of the other fish have these symptoms. Ammonia less then zero, salinity 1.024-1.025, ph 8.1-8.3. What could this be? Need help, please!
