Zoa's Dying from inside out

Hey everyone. We have just started adding stuff to our tank not to long ago. The tank has been up since end of last summer, so Septemberish. Noticed about a week ago our zoa colony closed and yesterday saw that it looks like it is almost dissolving from the inside out on some of the polyps leaving a clear shell. Upon seeing that completed a water change and then today tested tank and every thing checks out ok but our nitrate level is a little high at about 20ppm. Here is a picture of it. Just curious if the nitrate level alone could do this or is there another cause for it? Going to do another water change either tomorrow or day after to try and bring down the nitrates.

Nitrates at 20 are fine. That's not the problem.

In order to help you, we need to know exact numbers phos, nitrates, sg, alk, calcium, mag, tank size, lighting, filtration, inhabitants... You get the picture.
