sick clam or healthy?

Hi, all my maxima does not seem to want to attach. He has been in the tank for 4 weeks. Every once and a while he goes on a stroll. I got a chance today, to watch while the foot came out. Excuse my non scientific terms, but what I saw was a central shaft that was out about 1 inch, then I watched a larger surrounding column come down around it and push. It was cool. Not sure where he is trying to go. However I also noticed that the side of the mantle? closest to me appears torn. What is the big opening (is that gaping?) Could someone please look at the pic and tell me is this normal or not?

i have 4 clams, and 2 have never attached to anything, and are healthy. IMO, them attaching to something doesnt always mean theyre happy.
