had the same happen to me with 3 clams. found 3 large clam worm in my tank that drilled through the soft spot on the bottm of the clam. Found them when I shine with a flashlight at night, they came out of the clam. They were irridescent tan and about 8-10 inches long and had many small bristols (single row), but not like a bristol worm. I got them out by leaving the clams in and at night grab them with a long tweezer and flashlight.
