Question Is my fish sick?

I don't have a photo but I just want to get your feedback before I peruse this further.

So I have 5 Damsels in a 72 gallon tank. Tank has good water conditions as I can tell (0 ammonia, 0 nitrite, 5ppm nitrate)

I noticed the other day that one of my damsels has a "damaged" dorsal fin (looks frayed). When I noticed this I thought he may be sick but then I realized that this is the fish that all the other damsels pick on (chase and nip at) and thought that he probably had a battle wound.

Today I noticed what I am going to call a red "abrasion" under his mouth.

He doesn't have any white spots on him otherwise .. and his color is good. He is eating well .. and doesn't ever scrape himself intentionally on the sand or rock work,

Any thoughts?
