Tough to tell from the pic. It seems very localized, and very white...too much so to be HLLE. Perhaps something injured it? I had a crab attack a wrasse leaving a white flesh mark...
Is it three dimensional? Flat? Fluffy?

I also wonder if he ran into something, like a tight hole. Mine has darted into a edge of a rock and got a good scrape on his side. But do not know if this is your problem.
