What does a 10% water change include?
When calculating how much water volume a 10% water change is, does it include the main display or main display, sump and refugium?

It's normally calculated by guessing total water volume of the system. I say guess because it's pretty hard to determine volume after the fact.

I totally agree with Agu. It would be total water volume of entire system...display + sump + fuge. Like Agu said, a guess since it's usually hard...at least for me....to figure out totals gallons after displacement. I've always just went by the actual sizse of the tank.
