When to add Ick treatment??

I've been treating a sick Flame Angel with Kordon Ich-Attack with good results so far. What surprises me is that I find new spots appearing on him after others have gone away. I'm trying to educate myself on this so treatment will be effective.

According to what I've been reading about the life cycle of Ick, it is most effectively treated in the "free-swimming" stage. Can I assume that this stage is when the white spots have disappeared because they have hatched open, or is the presence of white spots also part of this stage? I have to ask because of the fact that the spots re-appear. Could it be that I'm not medicating at the most opportune time? This medication also gives me the option for 2 treatments per day and I'm trying to develop a strategy for timing the second dose. Should I just add dose #2 when I notice the spots gone, or is there a better way?
