T5 bulb combos to match MH bulb color: Thread

I have been researching a lot lately about T5 bulb combos and people are always asking "I am switching from X MH bulb to T5s and want a T5 bulb combo to match my old MH bulb color output." Lots of people give lots of combos they have found to match different MH bulbs. This info is scattered throughout this forum and is very difficult to find, especially in a few long threads.

So I wanted to start a thread of people who have switched from MH to T5s and attempted to mimic a MH bulb. Please list the following:

Fixture/Ballast
Size bulbs
Number of bulbs
Types of Bulbs
Placement of these types from front to back

I, personally, am looking for a 6 bulb combo to match a 14k Phoenix, but all bulb combos are good as I want this to be a thread people can turn to. Thanks.
