Coco Worm leaving tube

I know these worms sometimes leave the tube they are in and usually not a good sign. Does anyone have any ideas on what to do or just leave it.

The other question is feeding. I've been feeding cyclop-eze and read that this is too big. Is phytoplankton the way to go?

This is the first time it's done this and I've had it for over 2 years.

Any help would be appreciated.
