My Kole tang has Hole in the Head disease, other fish are acting strange

I have had my kole tang for about a year now. It has developed hole in the head. It is still eating and my water parameter are in check. I have no measurable nitrates and I feed spectrum pellets.

I read up on the disease and there is no real conclusive evidence of what causes Hole in the head disease.
Here are some possiblilties
Water purity (Can't be, My water has 0 nitrates and has low phosphates)
Lack of nutrition (Possible, all I feed is spectrum, I will try some nori)
Introduction of carbon source (I am leaning more towards this, I have been dosing vodka for about 2 months and I just started agressively using GFO)

Another note. SOme of my fish are acting pretty wierd. My female blue Jaw has not came out of the rocks for a couple of days. My female leopard and bannana wrasse are staying in the sand. I stirred up the sand today and the leopard came out and ate some food.

On the other hand my clowns, pajama cardinal, and my candy hog act completly fine. My kole acts the same it just looks bad. Poor thing.

I am leaning more towards the Introduction of carbon source I am thinking about stoping vodka and going to MB7 and GFO. Or do you think it is GFO?

Also I forgot to mention I just redid my pluming on my tank. I went from vinyl tubing to PVC. I used PVC cement and let it dry for about 30 minutes. Once it dried, which doesn't take long, I started my return pump. I really don't think it is this becaue my corals look great and polyps are extended like they should be.

What do you guys think????

I've heard stray electrical current can cause HLLE, have to tested for that? You might try a search for how to test if your tank has any stray current from your equipment here on RC.

Yeah, could be another possibility. There are so many possibilities. Sounds like no one knows the real casue.
Plus my corals look great, wouldn't stray voltage effect tehm as well?
