Id Please, Coral in Trouble



Can someone please id this coral, I think LFS said it was from Austraila?
I'm losing some polyps

Calc is 550, Alk 12.6 dkh, Mg was over 1400
Salinity 1.025
Nitrates 15-20 ppm, I'm trying to lower
Phosphate .015 ppm

Calcium reactor NOT dialed in and my testing discipline caused the High values.
I'm working to bring these things down with 15% water changes weekly, then start testing for consumption then dial in calc reactor.

Meanwhile,

Once this coral starts shedding skin, is it on it's way to certain death?

Anything I can do?
