ID Coral
I bought this frag from a local reefer. He identified it as a torch coral. After looking again on liveaquaria.com their pictures look different compared to mine. Mine has green tentacles and purple tips. The torch pictured online has green tentacles and light green tips. I thought maybe this was an elegance or maybe a frogspawn?
I appreciate the help.

+1 for "frogspawn" - Euphyllia sp.
It also doesn't look healthy at all. Looks like it had recent polyp bailout on the other two heads. At least that one head is open.

Thanks, I will be babying it for a while. I almost left it behind, but I thought it looked interesting. The few things I put in my tank have done really well, I'll work on this one. Do you recommend anything to help get it back healthy? I did soak in Iodine, anything else?

Its a branching hammer coral- related to both frogspawn and torch. The iodine soak should be plenty as far as disease preventative IMO. Just let it hang in a place with moderate light and current; feed it on occasion to speed up recovery. Small bit of mysis, brine shrimp, or something like Rods Food should work well. Just give it time and hopefully itll recover.

I agree with the Branching Hammer. The tips are elongated like a 'hammer'.
Keep it in low flow while it recovers. Mine spreads out nice a full in lower flow. Stable water parameters and a good light source should help it come around.
