Porcupine Puffer has small white spots

I have a tank that has had fish and coral die multiple times but we have changed out the water.

I have a porcupine puffer that has tiny white spots all over his body and fins and larger patches on his eyes. Most of them are smaller than a grain of salt.

Any thoughts as to what it is?
