What direction is right-side-up? I always thought it grew from narrow to wide, but the photo on brs shows differently. I see it stacked in both directions in people's tanks.

It's dead coral, so if it were alive it'd grow the way you describe it -- narrow to wide. However, now that you're using it as live rock, you can place it any way you like, since in a reef, the dead coral could've broken off the reef, tumbled, and may be upside down, and now new coral is growing on it. That said, I agree with you that BRS shows it upside down IMO, since in my tank I try to keep all of the pieces flat (wider) side up. I personally think it looks better, and allows for less contact with the bottom of the tank, and creates more natural caves.

It depends on how you want to use it, too. I used Pukani as the base rock (Reef Saver for the rest), and placed it all on the side, pointed inward. It still looks great, and has more area for caves than if I had placed it wide side up. My trigger and puffer continually move through it, and the snails and hermits all use the smaller caves to hang out.
