Frogspawn Losing Heads??

Hello,
I've had a frogspawn for a few months now. When I bought it it had four heads, one of them was dead (the LFS discounted it on account of that) but it seemed to be doing well otherwise. Over the first couple months, the second head on that same branch died, I chalked it up to someone wrong with the branch. A few days ago, the third head has now receded and died!

It seems to have tons of new growth on it, like it's about to sprout out more heads, and yet this one just died. Does anyone have a suggestion about what the culprit may be, or if it looks sickly in general? All tank parameters are normal, and the hammer I have looks amazing. Are those little branch looking things on the skeleton new growths, or something more sinister?

Really hard to see anything on there... maybe some vermetid snail shells?

For reference, here's a pic of mine from some time ago when the first new head was starting.
