Urgent Nem help..

I've got at 90 gallon display..49 gallon fuge..
Tanks been up and running about a year and a half..ive had my LTA for nearly a year now..thriving for the most part.. with a few time it became unhappy for whatever reason...
Parameters
Alk 8.5
Cal 420
Mag 1390
Phos. .5
N03 0

Nem took a turn for the worse last week...it was fine up until then..
It closed up and started crawling out of the sand like it wanted to move.. but Instead just crawled out onto the sand and has been just laying there, closed almost all the way up... just laying there..

Last night before lights out... it was looking very bad... I snapped a picture.. I check on it this morning.. it looks even worse.. and is expelling its guts.. I'll snap a picture..

Is there any way I can save it? It's been thriving for a year...and just crashed.. nothing new has been added or changed in months.. the rest of my tank.. as well as my rbta is all thriving..

Any advice is very much appreciated and needed... I'm not sure what to do

Seems like something changed. Lights, salinity, temp,pH, flow.. the numbers you posted seem fine of themselves but are they stable? The po4 is high but if that's how you normally run your tank dropping it will absolutely shock your nem espically if it's done fast. I keep my po4 0.04-0.06 btw. To over simplify, the zooplankton could use po4 or light for food. If your po4 is high that type of zoo will out multiply the light "eating" ones so you have to slowly phase it back to using light for food vs. phosphate or risk photoshock. If this is a drop you could do a light acclimation by dimming or screening. My lta doesn't like changes for better or worst. Its generally happy with stable and well fed. do you have new water on hand to do a water change? If that po4 isn't usually that high I'd double and triple ensure the water matches existing parameters and do the largest change you feel comfortable with. Po4 swings in general either way really mess with nems so keep that stable where ever its at normally.
Maybe you can turn flow around it down or off for a short time to encourage reattachment so it doesn't get blown around? Obviously if you try messing with flow watch to make sure this doesn't causes more stress but this is usually how you let them attach initially.
If your po4 is normally high you could attempt a water change just in case you somehow got something in the tank like a cleaner, perfume, kids hand just make sure not to drop the po4, match the existing normal as close as possible. I'd add defrosted cube "juice" to the new water drop by drop to accomplish this personally.

My p04 generally run a between .05 and .1

Sorry just noticed that typo.. p04 is at .05 atm... not .5

I check it often with a Hanna checker just because it's pretty easy lol

Perams are stable..

I am making water now for a decent water change... I will do 40 gallons..

Lights have been the same for months.. haven't tested ph in forever.. It's always been a constant 8.. I run an upflow ats with a reverse light cycle to keep ph the same at night..

That's an awesome idea for matching p04... I will have to try that..

I started running some carbon today.. and will run some pura filtration pad (similar to polyfilter except with gfo and carbon impregnated in case there is anything in the water..

Kids are grown and out of the house now..so that option is covered..

Oh..tank is a relatively low flow tank.. due to the LTA not liking my flow being much higher

Ah that makes more sense then but not why the nems acting odd. I'd definitely get on a water change. When on doubt wc lol. Mine hates flow too, it's in a private 'species specific' tank with the red saddleback clowns it hosts (their jerks) and a few soft coral. all power heads are pointed at the sides of the tank lmao. Have you been feeding this nem at all, how much, what, and frequency.
I seen cases where people feed 2-3 times per week which is way, way too much.
Just a thought...

In addition, if your not already doing, run activated carbon to soak up any contaminants that may have found there way into the water.

I feed it about once a week... or when it tells me it's hungry...
Small krill or pieces of store bought frozen shrimp..

Started carbon a few days ago when the Nem started acting funny

It's still alive.. It's holding onto the rock tight..but something are some of its tenticals last night...

Should I just keep monitoring and cross my fingers...

Or should I step in and end the poor nems suffering.. :-(

Her clown is trying so hard to help.. trying to dig it a hole in the sand. Trying to push it back to the sand..and guarding the Nem pretty feircly

I'm lost..dunno what to do

This is so sad. I dunno what to tell you to help that you haven’t already tried.

I lost a LTA in almost exactly the same way. The worst part was the clown trying valiantly to help.

I finally re-homed nem w a friend who is much more experienced, had a dedicated clown/nem tank. He couldn’t save it, either. sigh

I'm torn between waiting and crossing my fingers. Or doing the humane thing...
She was literally perfect on day... then fast decline the next

Me too...and I am about to...
My avatar is a pic of my LTA and her clown last year... deff my favorite animal in my tank.. and it was SOOO big when happy.
Almost 10 inches diameter..


It scares me as well because I am currently done with the planning and purchasing for my new 74 gallon ckownfish/Nem tank I'm gonna build very soon..

I'd be so torn trying to diffuse lights and flow to get her to reattach and waiting untill the last min to euthanize if I have to.
I really hope you don't have to do it but it just doesn't seem good
I really hope your nem has an amazing recovery that's so sad :'(

I would not know why it died.
I have seen a number of Nems turn inside out when feed too much too often or too big.
Feeding is not required at all, but at most, once per month, small fingernail size piece of shrimp. They feed from light.

I just lost my LTA. I think I had an overfeeding. Small piece of scallop. about the size of a penny. Must have been too much. It deflated with the lights on. Then it got pulled into powerhead. After that it did not move. I am thinking too high of flow and overfed. Hard Lesson learned.

I am sorry to hear that.
All Nems require perfect water, strong lighting, moderate flow, and That is all.
All feeding will do is make it grow large, maybe divide, a nem takes days sometimes to fully digest even small pieces...
