Toadstool disintegrating

A few weeks ago I noticed the stalk of my toadstool was wasting away. I saw a few little chunks of flesh were beginning to detach and float away. This evening while I was trying to re-position it to try and make it happier, the head broke off. Should I try and replant it somewhere or toss it and hope the stalk re-grows. I don't know the current parameters but I have several other softies in the tank and they all look fine.

If the head is in good shape on the underside, then I would place it in low flow with good lighting on top of some rock. it will attach and a new stalk will regrow if the coral is in good health.
