Receding corals and ugly anemones

I bought an Orange Bulb anemone a few months ago from my place of employment, and it has done nothing but bad in my tank. It went from a stunning pink/orange, to a pastey white. My clown is still in there and it still has buls, just not the color. I have a 25 gallon with 4.4 wpg. My yellow polyps disappeared and the only thing doing well in there is my frogspawn and hammer. Any suggestions? Thanks

I just checked my water. Nitrate-.20, Nitrite-.00, Alkalinity-180, PH-8.4, ammonia is showing a tiny tiny bit, about .2, but i thnk that is due to a lack of water changes in 3 weeks. Salinity is at 1.024 about 78 degrees . im running a skilter filter with the skimmer only running on sundays due to the amount of bubbles it puts into the tank. Rio 400 powerhead in the corner putting quite a bit of flow into the water. I have some pictures how can I put the on here?
