Question acclimation, qt, and FW dips

Ok, there is soo much info on QT, formalin dips, freshwater dips, hypo etc... So, lets say I setup a QT and filled with tank water. Fish arrives, I suspect it will have ich (lets say it is a tang), what do I do? Acclimate to QT water and then do freshwater dip? Or freshwater dip first then what? I am confused....

I know that there are many that prefer not to do any dips. But I like the fact that freshwater dip will remove some parasites currently tormenting fish.

What do you guys/gals do when sick fish arrives? What is the procedure?
